With Diablo vanquished, the hosts of the High Heavens set to rebuilding the ruin of their home. Tyrael rejoined the Angiris Council as the mortal Aspect of Wisdom—a quality he knew would be sorely needed in the days ahead. He strove to heal the pain and distrust that spread through Heaven in the wake of Diablo's invasion, and to forge a true alliance between angels and mankind…. After Diablo was defeated and trapped in the Black Soulstone, the Angiris Council resolved to secure it within the High Heavens, but Tyrael doubted this solution.
He recruited powerful humans from all over Sanctuary and dubbed them the new Horadrim, the first since the death of Cain. With their help, Tyrael stole the Black Soulstone from the High Heavens and planned to conceal it beneath the ruined city of Corvus.
Malthael, the former Archangel of Wisdom, had become tormented by the mysteries of creation. He had abandoned his brethren and, in his search for the nature of souls, had become the Aspect of Death.
After the nephalem defeated the Prime Evil, Malthael decided to exterminate all beings of demonic heritage—including humanity—to end the Eternal Conflict. He attacked the new Horadrim and seized the Black Soulstone. His army of Reapers, a terrifying host of angels and undead, began to slaughter the people of nearby Westmarch and harvest scores of souls. Here, Malthael would reshape the Soulstone to consume all demonic essence in Sanctuary. With the help of a crusader of the Zakarum faith, the nephalem confronted Malthael.
In combat, Malthael shattered the Black Soulstone and devoured the released soul of the Prime Evil, devastating the battlefield with the powers of Hell and Death. After a grueling conflict, the combined might of the nephalem overcame the Aspect of Death. Malthael vanished in a blast of energy, releasing all the souls he had consumed—including Diablo.

At the first catapult, talk to Sergeant Burroughs. Go Northwest a little and kill the demons attacking the barracks. Once you kill the demons and free the workers, they'll start to raise the catapult themselves.
Just stand beside them and protect them while they do their thing. The less that die, the faster it'll raise. Once you finish here, continue to the second catapult. At the second catapult, talk to Lieutenant Lavail. He'll tell the the workers in the barracks to come out and start doing their jobs. Same as the first one, protect the workers, just there are more enemies this time.
After you finish with the second catapult, head onwards to the third catapult and talk to Lieutenant Clyfton.
There won't be any workers helping you with this catapult. It'll just be you, Lieutenant Clyfton, and tons of demons. You can click on the winch every few seconds, but don't forget to kill the demons that spawn as they'll continue to spawn until the catapult is raised.
Head Northwest and kill the demons attacking the stronghold, then enter from where they came, The Keep Depths Level 1. Explore this area until you find The Larder. This is your first main boss to fight in this Act. Ready yourself and enter. He has two main abilities he uses. He summons little yellow gobs like the ones here and he puts clouds of poisonous gas on the ground at your location from time to time that last for a while. If you take too long to kill him, he'll increase his rate of spawning poisoning clouds and you will eventually not be able to move without running into a cloud.
Head back to the stronghold and go Northeast, straight across the bridge and into The Arreat Gate. Explore the area until you find the Bridge of Korsikk and cross it. Explore the next area and find and destroy the three Demonic Ballistae and the Trebuchet.
Their locations are on the map shown here, or at least one possible set of locations if its randomized every game. Head Northeast until you reach Rakkis Crossing. Tyrael will be here. Talk to him and then continue following the path.
There is no long quest or list of requirements to find it. Simply talk to Zoltun Kulle in Act town and then travel to the The Ruins of Sescheron in Act 3 and find the Elder Sanctum, somewhere on that map you will find the cube. I found it in the Elder Sanctum map area.
